New Patients
WELCOME TO OUR PRACTICE​​

On the day of your first appointment, we ask that you arrive to the office 15 minutes prior to your appointment time so that we may complete the registration process and the nurse may review your health history. Please complete the New Patient Forms that are available under the FORMS tab and bring them with you to your first appointment along with your current insurance card, a valid government issued photo identification, a complete list of medications and vitamins/supplements that you are taking to include medication name, dosage, and directions. If your insurance requires a referral or co-payment, it will be required for you to have these when you check in.

Our office accepts patients that are considered self-pay. Self-pay are patients without insurance coverage, patients covered by insurance plans in which the office does not participate, or patients without an insurance card on file with us. We do not accept attorney letters or contingency payments. It is always the patient’s responsibility to know if our office is participating with their plan. Self pay patients are required to bring a deposit of $50.00 at the initial appointment and will be required to make payment arrangements for the remaining balance. Extended payment arrangements are available if needed. Please ask to speak with a billing coordinator to discuss a mutually agreeable payment plan.

Self-Pay Obstetrical care patients will be required to pay $500 down PRIOR to their first appointment with the physician and $125 monthly until delivery. This payment arrangement helps to pay for your Obstetrical care and delivery but does not cover the entire balance. Any remaining balance will be billed after delivery and will need to have a regular monthly payment plan set up. There may be additional expenses that are not included in this agreement and will need to be paid at the time of service. Our Financial Policy is available under the FORMS tab. It is never our intention to cause hardship to our patients, only to provide them with the best care possible and the least amount of stress.
